Please complete this application form for your child regarding peripatetic music or LAMDA lessons.
By completing the form you agree to:
- Ensure that your child is prepared for lessons with their instrument if appropriate
- Ensure that your child makes every effort to attend scheduled lessons and to give as much notice as possible if they are unable to make a scheduled lesson
- The details on this form being passed directly to the relevant peripatetic teacher
- Ensure regular practice takes place at home
- Give half a term’s notice in writing should you wish to terminate lessons

Upon completion of this form, Mrs Golchin or the peripatetic teacher, will contact you directly in order to arrange the lessons.
