Please complete this form to arrange for your child to start LAMDA lessons.
By completing the form you agree to:
- Ensure that your child is prepared for lessons
- Ensure that your child makes every effort to attend scheduled lessons and to give as much notice as possible if they are unable to make a scheduled lesson
- The details on this form being passed directly to the relevant peripatetic teacher, in order for lessons to be booked
- Give half a term’s notice in writing should you wish to terminate lessons
- Ensure regular practice takes place at home.
A block of lessons costs £110 per term, which usually consists of 10 lessons.
Upon completion of this form, Mrs Golchin or the peripatetic teacher, will contact you directly in order to arrange the lessons.
