Partnership Reference Form

Supporting Your Child’s Senior School Reference

To help us provide a full and well-rounded reference for your child, Pennthorpe works in partnership with families to ensure that each child’s reference reflects who they are as a person, including their personal qualities, skills, interests and achievements both in and outside school.

We are very grateful to our families for helping us to build this picture. Please complete the sections below with as much relevant information as you can.

This form can be updated at any time using the link sent to you after submission. We encourage you to return to it throughout the year to add any new activities, achievements, responsibilities or experiences. All information shared will help us prepare your child’s Senior School reference.

"*" indicates required fields

Background

Please include a summary here of your child’s family background, health/medical needs, attendance, schools attended (with dates), siblings, bursary support, and any other circumstances of which a senior school should be aware.
Please tell us about activities your child participates in outside school on a regular basis. This could include sports clubs, music lessons, drama groups, dance schools, Scouts, Guides or Cadets, community or religious organisations, academic clubs, or other hobbies and interests. For each activity, please provide the organisation or club name, how long your child has participated, how often they take part, and any positions of responsibility they hold.
Please provide details of any awards, certificates, qualifications or notable achievements your child has gained outside school. These might include music or LAMDA examinations, sporting or dance awards, scholarship programmes, academic competitions, coding or language qualifications, or national awards and certificates. Please include dates and grades or results where possible.
Please tell us if your child has been selected to represent a club, county, region or national organisation. This could include sports representative teams, county squads, national competitions, orchestra or choir selection, performing arts productions, or academic competitions. Please include the dates and level of representation where possible.
Please provide details of any responsibilities your child has held outside school. This could include team captaincy, house or club leadership positions, young leader awards, volunteering, charity work, community service, or mentoring younger children. Please include the organisation and the duration of their involvement where possible.
Please tell us about anything else your child does or has achieved outside of school which we might not know about but which may support their application to a senior school.
Please tell us about any significant activities, interests or experiences that may not fit into the sections above. This could include personal projects, entrepreneurship, fundraising initiatives, public performances, expeditions, cultural or creative activities, specialist training programmes, or other extended commitments or achievements.
